Recurrent Diabetes Mellitus

Electron microscopy of kidneys transplanted into diabetic individuals can show increased glomerular volume within 6 months, and increased glomerular basement membrane thickening within 24 months. Sclerosis of 10% or more glomeruli has been recorded within 2-3 years of transplantation. Hyaline arteriolosclerosis can be demonstrated in nearly all cases by 4 years. Immunofluorescence examination may show linear deposits of IgG and albumin in the glomerular and tubular basement membranes. The earliest reported nodular glomerulosclerosis is 8 1/2 years after transplantation.
