This biopsy showed severe interstitial lymphoplasmacytic infiltrates that also featured scattered neutrophils and eosinophils. There was tubular disruption i.e. grade 3 tubulitis, and the biopsy met the criteria for the diagnosis of Banff type 1B T-cell mediated rejection. It contained scattered enlarged hyperchromatic tubular epithelial nuclei containing viral inclusions. In this photomicrograph the inclusion takes the form of an intranuclear mass of basophilic material (aggregated viral particles) that result in peripheral margination of the cellular chromatin.
