The earliest changes associated with the onset of acute viral type B
hepatitis in a liver allograft are usually found within the lobules.
There is
disarray, Kupffer's cell hypertrophy, spotty acidophilic necrosis of
hepatocytes and lobular inflammation of variable intensity.
The inset shows staining for HBcAg. Note the nuclear and
focal cytoplasmic positivity.
