A separate dense area of mononuclear inflammation. Particularly in a patient with known PTLD, such uniform inflammation, without evidence of other cell types such as eosinophils or neutrophils, would raise the question of PTLD vs. rejection. The overall appearance of this case was in keeping with rejection. Immunocytochemical stains for EBV were negative.
