Immunostain for C4d (20x) on paraffin-embedded tissue showed diffuse uptake in peritubular and glomerular capillary endothelial cells. It is not very common for the fixed tissue specimen to show such striking positivity, even in the setting of humoral rejection, and allowances are made in the Banff system for the usually weaker staining relative to frozen section samples.
