PATIENT HISTORY: The patient is a 36 year old white male. History of double lung transplant for cystic fibrosis. History of ACR grade III, in 8/96. Decreased breath sounds in right lung otherwise stable with scattered wheezing.

Final Diagnosis (Case 1)

ALLOGRAFT LUNG, SITE UNSPECIFIED, TRANSBRONCHIAL BIOPSY -
  1. MILD ACUTE CELLULAR REJECTION WITH LARGE AIRWAY INFLAMMATION. NO SMALL AIRWAYS PRESENT TO EVALUATE BRONCHIOLITIS OBLITERANS (A2c/A2B2).
  2. ALVEOLAR HEMOSIDEROSIS WITH INTERSTITIAL CALCIFICATIONS.

Gross Description - Case 1

The specimen is received in saline labeled "transbronchial biopsy". Received are four irregular fragments of tan-pink, soft tissue, these range from less than 0.1 to 0.1 cm. in greatest dimension. Additionally received are five irregular portions of clotted, dark red-brown blood. These range from 0.1 to 0.4 cm. in greatest dimension.

The microscopic examination substantiates the diagnosis.
