PATIENT HISTORY: The patient is a 63 year old male status post orthotopic heart transplant.

Final Diagnosis (Case 1)

ALLOGRAFT HEART, ENDOMYOCARDIAL BIOPSY -
  1. FOCAL MILD ACUTE CELLULAR REJECTION (GRADE 1A).

Gross Description - Case 1

The specimen is received in buffered formalin labeled "endomyocardial biopsy tissue" and consists of four irregular portions of tan to white soft tissue, ranging from 0.2 to 0.3 cm. in greatest dimension.


Microscopic Description - Case 1

Three fragments of myocardium with a mild mononuclear infiltrate in an endocardial, perivascular and interstitial location. Myocyte injury is not identified. No infectious agents seen. Endocardial and interstitial fibrosis is present.
