PATIENT HISTORY: The patient is a 62 year old white male status post left lung transplant for IPF.

Final Diagnosis (Case 2)

ALLOGRAFT LUNG, LEFT LOWER LOBE, TRANSBRONCHIAL BIOPSY -
  1. MINIMAL ACUTE CELLULAR REJECTION WITH LARGE AIRWAY INFLAMMATION (A1c/A1B1).
  2. ALVEOLAR HEMOSIDEROSIS.

Gross Description - Case 2

The specimen is received unfixed labeled "transbronchial biopsies from left lower lobe of the lung" and consists of nine somewhat ovoid, smooth, soft, pink-grey and hemorrhagic gelatinous tissue fragments, ranging from less than 0.1 cm. to 0.2 cm. in greatest dimension. The specimen is entirely submitted labeled ATB.

The microscopic examination substantiates the diagnosis.
