Pretransplant Donor Biopsies

Pretransplant Donor Bx
Figure 9. A biopsy showing predominantly mononuclear, but mixed portal inflammation is a common finding when evaluating donor organs. This biopsy also shows mild cholangiolar proliferation, and focal extension of the infiltrate into the lobules, but active hepatocyte necrosis is not noted. These histopathological changes can be observed when the donor experiences sepsis, or a prolonged stay in the intensive care unit before the liver is harvested for transplantation. However, in such cases, it is first important to exclude the possibility of chronic viral hepatitis infection.
